|All prizes must be picked up at the office (unless otherwise specified) of KOLA 99.9 located at 1940 Orange Tree Lane Suite 200, Redlands Ca 92374 unless otherwise specified upon point/entries redemption.
Station hours are 8:30am to 5pm, Monday through Friday (except holidays). When picking up your prize, you must provide a valid California Driver’s license or a government issued photo ID card, your prize confirmation print out and your social security card (if expressly stated). In addition, you MUST fill out a station provided tax/prize release form before your prize will be awarded.
If a winner is unable to personally pick up a prize, said winner can assign a proxy to do so by sending a clear readable copy of their California Drivers License or government issued ID card, an original and complete tax/prize release form (provided to the winner in advance by the station) along with a signed and detailed letter of release authorizing a “specific person” to pick up a “specific prize”. All signatures on these documents must clearly match the signature on the government issued ID. Furthermore, the proxy must provide proof of their identity (in the form of a valid California Drivers License or a government issued photo ID card) as identified in the release letter provided by the winner. If any of the proxy documents provided are suspect for any reason, Anaheim Broadcasting retains the right to delay release/delivery of the prize until the documents provided can be verified.
If acting as proxy for other KOLA ONLINE ADVANTAGE Club Program members, you may only pick up prizes for up to 3 people TOTAL per day.
Prizes must be picked up within 30 days of purchase – unless otherwise specified. Prizes not picked up within 30 days from the date of purchase will be forfeited and points/entries will not be refunded.
Mailing options are at the sole discretion of KOLA 99.9 and will only be considered in extreme circumstances, or will be based on the prize size, weight, timeliness and value. Prizes valued at over $15, time-sensitive prizes, tickets and gift certificates must be picked up. Member is always responsible to keep his/her KOLA ONLINE ADVANTAGE CLUB account up-to-date with current information. If KOLA 99.9 deems that an individual has extreme circumstances and a prize is mailed out, KOLA 99.9 is not responsible if it is lost, stolen, misdirected, or damaged. The prize will not be replaced with future prizes nor will points/entries be refunded.
Prize winners are responsible for any and all expenses associated with their travel to and from the KOLA 99.9 offices or other any location(s) associated with claiming a prize or award.
KOLA ONLINE ADVANTAGE CLUB points/entries will not be credited or refunded for any reward, item or certificate which a Member claims but does not subsequently use.
Member agrees to provide only true and accurate information to the KOLA ONLINE ADVANTAGE CLUB Program at all times. In conjunction with claiming any item associated with the KOLA ONLINE ADVANTAGE CLUB Program or spending any points/entries for items, Member agrees to verify that all information in their KOLA ONLINE ADVANTAGE CLUB account profile is up-to-date and accurate by providing a valid California driver’s license or government issued photo ID that matches the information in their profile. Furthermore, regular profile updates are strongly encouraged.
KOLA ONLINE ADVANTAGE CLUB award items are not transferable. Awards may not be substituted for cash or any other items or merchandise.
All award claims are subject to verification by KOLA 99.9 Management. All decisions made by KOLA 99.9 Management are final.
Prizes and awards will be awarded and distributed at the discretion of KOLA 99.9.
Prize is non-transferable.
Must be 21 years of age or older. Prize may be used only for the date(s) specified. Hotel guest room accommodations for winner and guest, double occupancy, three days and two nights based on availability. Not redeemable for cash, not transferable, may not be combined with another offer. Winner must use all prize components in a single visit. Winner may not substitute prize components. In the unlikely event that one or more of the prize components is not available for any reason, Downtown Grand Hotel & Casino/The Mob Museum reserves the right to substitute a Las Vegas experience of equal or greater value at its sole discretion. In the unlikely event that the Museum is closed during the scheduled trip, for any reason, Downtown Grand Hotel & Casino/The Mob Museum may substitute a Las Vegas resort experience of equal or greater value at its sole discretion. Downtown Grand Hotel & Casino/The Mob Museum are not responsible for transportation. Gratuities are not included. Hotel guest must present a major credit card at check-in to guarantee hotel charges not included in this prize.
