KOLA 99.9 FM
GENERAL CONTEST RULES (As of February 25, 2010)


The following are basic contest rules for any and all contests run on KOLA 99.9. Each contest may have its own special qualifications and eligibility rules, in which case a separate addendum to these rules will be available.

a. All KOLA 99.9 contests are open to any listener unless specific age is required by type of contest, participating agency or advertiser or by law. No household can win more than a maximum of 1 prize in any given month. This includes any contest where listeners must qualify for a grand prize (i.e. qualifying will count towards the 1 prize maximum. All qualifying prizes must be claimed at the KOLA business office - 1940 Orange Tree Lane Suite 200, Redlands 92374 - within 30 days. KOLA 99.9 is not responsible for the inability of the listener to redeem concert tickets, movie tickets, theme park tickets or the like due to circumstances beyond our control such as cancellation of events, rides or showings of any kind. No substitutions will be made if the tickets or passes cannot be used by the winner. Prizes are not to be sold under any circumstances.
b. Each winner must provide The Company with a current, valid government issued California identification and a valid taxpayer identification number or social security number before any prize will be awarded. Any person winning over $600 in prizes from The Company will receive an IRS Form 1099 at the end of the calendar year and a copy of such form will be filed with the IRS.
c. Any contest that involves KOLA 99.9 choosing a winner from entries, such as email contests ("Muffin Monday", "Office Of The Day" etc.) or any "KOLA Online Advantage Club" contest where entering through the KOLA Club section of the KOLA website is required, will not count towards the 1 prize maximum in any given month.
d. There may be contest(s) where you can only have one winner per household per year. Additionally, there maybe contests where a listener may only be a contestant once per calendar month (even if that listener didn't win).
e. Unless otherwise specified, all contests are conducted during general programming hours and contests are not conducted during programs that are carried through syndicated sources whether "live" or pre-recorded that are not in control of KOLA 99.9. During hours that carry a syndicated program (non-general programming hours) contest rules and or the mechanics of operating any contests are void and no winners will be accepted during these times.
f. Winners may be required to sign a release, and all winners agree to allow KOLA 99.9 to use their name and/or likeness for promotional purposes without further compensation. All calls coming in to KOLA 99.9 are taped and may be aired on KOLA 99.9 without further approval.
g. KOLA 99.9 is not responsible for problems occurring within the telephone system which either inhibit or increase the chances of winning a phone-in contest. In the event two or more phone lines are "tied together" during a phone-in contest, the entire call and all callers "tied together" will be disqualified and another caller will be taken.
h. KOLA 99.9 can not award duplicate prizes to all parties in a "tied together" situation. In case of telephone line malfunctions, the decision of KOLA 99.9 will be final.
i. Employees and agents of KOLA 99.9, Anaheim Broadcasting and their families, their advertising agencies or representatives are ineligible to enter contests on KOLA 99.9. Winners may not request substitution or other consideration from the originally awarded prize.
j. All prizes, unless otherwise stated, must be picked up in person, and signed for by the winner at the KOLA 99.9 business office, located at 1940 Orange Tree Lane Suite 200, Redlands, CA. 92374, any weekday, between 9AM-5PM, within 30 days of winning. Holidays and weekends are included in this 30-day period. Designated winner's representatives will be allowed to claim prizes, provided prior arrangements have been made. If prizes are not picked up within the stated period, the winner waives their rights to the prize and said prize becomes the possession of KOLA 99.9 and may be awarded to another listener at KOLA 99.9 discretion.
k. KOLA 99.9 will not notify winners of time remaining on prizes to be picked up. It is solely the winner's responsibility to claim their prize within the time limit specified. No prize will be mailed unless specified during contest or if special arrangements have been made. Prizes are non-transferable.
l. KOLA 99.9 reserves the right to revoke prizes if the winner does not meet any or all eligibility requirements, even though an announcement may have been made on the air indicating that person was the winner.
m. KOLA 99.9 may, due to the unavailability of prizes awarded, substitute another prize of equal or greater value for the prize originally won.
n. KOLA 99.9 will not be responsible, or held liable for prizes awarded to an event that is canceled due to circumstances beyond our control. With respect to any disputes regarding contests, the decision of KOLA 99.9 will be final.
o. KOLA 99.9, its agency, and representatives absolve themselves and shall be held harmless from any action or liability arising from any KOLA 99.9 contest or prize awarded in a contest. All contest entries become the property of KOLA 99.9.
p. KOLA 99.9 may withhold any or all contest materials for any reason station management deems crucial to the completion of the contest, or for reasons relating to publicity and/or advertising. Unless otherwise stated, multiple entries by the same person will be discarded and may be deemed cause for disqualification. Additions or deletions to these rules are subject to change without notice.

KOLA 99.9 FM - OFFICIAL ENTRY RULES (as of August 21, 2014 2:53 PM)

Contest is open to U.S. residents 21 years of age or older, except employees and the immediate families of San Manuel Indian Bingo & Casino, KOLA 99.9 FM, Inland Empire Broadcasting Corporation, all other Riverside county and San Bernardino county radio stations, or their subsidiaries, divisions, affiliated companies, participating manufacturers and advertising agencies, the determination of which lies solely and exclusively with KOLA 99.9 FM.  Persons banned from San Manuel Indian Bingo & Casino, including self-bans, are ineligible.  Contest is subject to all federal, state and local laws and regulations and is void where prohibited or restricted by law.

There are two ways to participate; (1) Friday August 22, 2014 through Sunday September 21, 2014 (5pm), enter to win at www.kolafm.com no purchase necessary to enter; (2) Tuesday September 2, 2014 through Sunday September 21, 2014, listen to KOLA 99.9 FM.  Upon hearing the cue to call, the nineteenth caller to (909) 798-5600 will be considered a “qualifier” and win two admissions to the Final Event.  Additional qualifiers will be taken at random from the membership of KOLA’s free listener club, KOLA Online Advantage Club and from advertised KOLA 99.9 FM events.  Qualifier admissions are not transferable.

In the event that two telephone lines are tied together during contest, the entire call will be disqualified and another call will be taken.  KOLA 99.9 FM cannot award duplicate prizes to both parties in a “tied together” situation, which is a circumstance beyond our control.  In case of a dispute over telephone line malfunctions, the decision of KOLA 99.9 FM will be final. 

Only one qualifier plus one guest per household. Contestants may qualify only once.  In regards to KOLA 99.9 FM’s “Thirty Day Rule”, this rule is waived for the purposes of the Aloha Bingo contest.  To participate in the Final Event, all participants and their guest must join the San Manuel Indian Bingo & Casino Club Serrano, which is free, and be pre-registered by Wednesday, September 24, 2014 at 11:59pm to be eligible for the final event.

KOLA 99.9 FM and San Manuel Indian Bingo & Casino will host the Final Event, which will take place in the Yuhaviatam Room at San Manuel Indian Bingo & Casino, beginning at 7:30 pm on September 25, 2014.  Qualifiers must be present at Final Event to win any prizes. 

The final event will consist of ten games of bingo.  All bingo games will be played following the San Manuel Indian Bingo & Casino Bingo House Rules.  Each game will have a separate and distinct prize that will be awarded to the winner.  The prize for each game of bingo is as follows:  Games One through Nine – $1,000 cash, Game #10 - $10,000.  Only one grand prize of $10,000, and nine $1,000 prizes, will be awarded.  In the event of a tie in any game, the cash prize will be divided evenly between the contestants who have achieved Bingo. 

Winners will be paid by San Manuel Indian Bingo & Casino.  The odds of winning the contest will depend upon the total number of legitimate entries submitted. 

Additionally, a separate raffle will be held between games 9 & 10 to award a 15 day Hawaiian cruise.  Entries will be made up of all attendees who’ve successfully checked in and have been verified as a contestant through KOLA Promotions.  Only those verified as contestants will be eligible for the separate raffle.  Must be present to win.

One contestant’s entry will be chosen at random and announced as the potential winner.  The contestant’s name or identifying raffle number will be announced at least 4 times in 30 seconds.  If that contestant is not present (after the 30 seconds), then another potential winner will be drawn from the remaining entries.  This method will continue until a contestant in attendance is fully verified as the winner.

GRAND PRIZE (1): One (1) Grand Prize Winner will be eligible to receive one (1) 15-day Hawaii Cruise (embarking and disembarking from San Pedro, California), with Grand Prize Winner and Cruise Guest sharing accommodations in one (1) double occupancy balcony stateroom. ARV: $5,460.
TRANSPORTATION TO AND FROM THE POINT OF EMBARKMENT/DISEMBARKMENT FOR EITHER CRUISE IS NOT INCLUDED. All prize details not specified in the Official Rules will be determined in Sponsor’s sole and absolute discretion

Cruise includes all regular onboard meals and entertainment, applicable government fees and taxes for Cruise only. Cruise must be completed by September 30, 2015 or prize will be forfeited. All ground and air transportation, shore excursions fees, discretionary hotel and dining charges, meals in alternate dining venues, alcoholic beverages, boutique purchases, and any other incidental costs or other expenses not specifically set forth herein are the sole responsibility of Winners. Gratuities of $180 per person are the complete responsibility of the winner.  All reservations and cruise arrangements including, without limitation, the dates and times are at the sole discretion of Sponsor. All Cruises must be taken during the time period specified; no alternative cruise dates are available or Grand Prize will be forfeited. All prize details are subject to availability and restrictions. Cruise Guests must be 21 years of age or older as of the date of departure unless the companion is the child of a Winner. Minors must be accompanied at all times during trip (including, but not limited to, in-flight, hotel stay, cruise activities, and all other prize-related events) by minor’s parent or legal guardian. Winners and Cruise Guests must cruise on same itinerary. Cruise Guests must (or a Cruise Guest’s parent or legal guardian if a Cruise Guest is an eligible minor in his/her state/province/jurisdiction of residence) sign and return a cruise/publicity release before any ticketing of cruise occurs. The Winners and Cruise Guests must have all necessary identification and/or cruise documents (including but not limited to a valid passport, visa (if applicable) and all other necessary travel documents) required for Cruise. Cruise is subject to the terms and conditions of the Princess passage contract. As set forth in more detail in the passage contract, Princess reserves the right to refuse to embark any Winner or Cruise Guest, at any time if, in the sole opinion of Princess, a Winner or Cruise Guest, or any minor or other person in a Winner or Cruise Guest’s care during the cruise, are unfit for any reason for the Cruise. Sponsor shall not be responsible for any cancellations, delays, diversions or substitution or any act or omissions whatsoever by Cruise operators or any other persons providing any Grand Prize-related services or accommodations. Winners and Cruise Guests are also responsible for obtaining travel insurance (and all other forms of insurance) at their option and hereby acknowledge that Sponsor has not and will not obtain or provide travel insurance or any other form of
insurance. Lost, stolen or damaged Cruise tickets, travel vouchers or certificates will not be replaced or exchanged.

General Conditions: Prizes are non-transferable and no cash equivalent or substitution of prize is offered, except at the sole discretion of the Sponsor. If prizes, or any portion thereof, cannot be awarded for any reason, Sponsor reserves the right to substitute prize with another prize of equal or greater value. Prizes will only be awarded to addresses within the United States. Winners will be solely responsible for all applicable Federal, State, and local taxes connected with receipt of prize, the use and enjoyment of prize, or any other connection with this Sweepstakes unless otherwise required by law or unless this requirement is waived by the Sponsor in its sole discretion. Winners will receive IRS Form 1099 for the ARV of prize. As a condition of accepting the prize, Winners agree to furnish any requested information needed for tax reporting.

Winner must provide Inland Empire Broadcasting with a current, valid government issued California identification and a valid taxpayer identification number or social security number before Hawaiian cruise will be awarded. Any person winning over $600 in prizes in a calendar year from Inland Empire Broadcasting will receive an IRS Form 1099 at the end of the calendar year and a copy of such form will be filed with the IRS.

Ground and other transportation to and from the ship is not included. Any state and local government surcharges, and all taxes (sales, income, airport, value-added and other), gratuities, incidental expenses, licenses, registrations, telephone calls, fees or charges for changing dates, travel insurance premiums, transportation and other costs and expenses relating to prize and use or acceptance of these prizes are the sole responsibility of winner. Trip and travel insurance is not included.

All Official Rules must be complied with or prize will be forfeited. Contest winners must be at least 21 years of age. All taxes, surcharges and gratuities are the complete responsibility of the winner. Travel requires government issued picture identification. By participating in this promotion, all participants and prize winners agree that Princess Cruises, its partnerships, subsidiaries and affiliates and each of such entities' respective officers, directors, agents and employees are not responsible or liable for any injury, loss, illness, litigation or damage that may occur from participation in the promotion or acceptance, possession, use or misuse of prizes. In the event that any dispute arises regarding the meaning or interpretation of these official rules, participants agree that the dispute shall be resolved by applying the laws of California and that it shall be resolved by and within the courts of that state.

By participating, entrants agree to these rules.  Anyone disputing these rules may be disqualified by San Manuel Indian Bingo & Casino and KOLA 99.9 FM.  San Manuel Indian Bingo & Casino and KOLA 99.9 FM reserve the right to change these rules or cancel this promotion at any time.  San Manuel Indian Bingo & Casino and KOLA 99.9 FM are not obligated to provide a replacement prize(s) in the event the promotion is canceled.

In the event of a dispute, the decision of San Manuel Indian Bingo & Casino is final.  Winners are solely responsible for their own actions and agree to hold harmless San Manuel Indian Bingo & Casino, Inland Empire Broadcasting Corporation, KOLA 99.9 FM, and their subsidiaries, divisions, related companies, officers, directors, employees, and agents from any liability arising in connection with their participation and use of prizes.  Winner and other participants consent to the use of their name, image, voice, and/or likeness for publicity purposes in connection with the contest without prior approval or compensation by KOLA 99.9 FM and/or San Manuel Indian Bingo & Casino.

h. KOLA ONLINE ADVANTAGE CLUB points/entries will not be credited or refunded for any reward, item or certificate which a Member claims but does not subsequently use.
i. Member agrees to provide only true and accurate information to the KOLA ONLINE ADVANTAGE CLUB Program at all times. In conjunction with claiming any item associated with the KOLA ONLINE ADVANTAGE CLUB Program or spending any points/entries for items, Member agrees to verify that all information in their KOLA ONLINE ADVANTAGE CLUB account profile is up-to-date and accurate by providing a valid California driver's license or government issued photo ID that matches the information in their profile. Furthermore, regular profile updates are strongly encouraged.
j. KOLA ONLINE ADVANTAGE CLUB award items are not transferable. Awards may not be substituted for cash or any other items or merchandise.
k. All award claims are subject to verification by KOLA 99.9 Management. All decisions made by KOLA 99.9 Management are final.
l. Prizes and awards will be awarded and distributed at the discretion of KOLA 99.9.


4) Taxes
Any valuation of the prize(s) stated above is based on available information provided to The Company, and the value of any prize awarded to a winner may be reported for tax purposes as required by law. Each winner is solely responsible for reporting and paying any and all applicable taxes related to the prize(s) and paying any expenses associated with any prize which is not specifically provided for in the official rules. Each winner must provide The Company with valid government issued California identification and a valid taxpayer identification number and social security number before any prize will be awarded. Any person winning over $600 in prizes from the Company will receive an IRS form 1099 at the end of the calendar year and a copy of such form will be filed with the IRS. If the prize includes travel, lodging, entertainment, or a special event, winner is responsible for all taxes, expenses, and gratuities not expressly included in and as part of the prize. Winner agrees to accept all blackout dates, space availability requirements, etc. established by The Company's prize partner(s), such as hotels, airlines, trains, ships, etc. Winner agrees that acceptance of any trip taken as a prize from The Company does so entirely upon their own initiative, risk, and responsibility. If the trip requires traveling outside of the United States, the winner, upon winning the prize, must have a valid US passport. If the winner does not have a valid US passport upon winning the prize, the trip may be forfeited.


5) How to Redeem Points / Entries to Receive Prizes

a. A validated KOLA ONLINE ADVANTAGE CLUB Member redeems KOLA ONLINE ADVANTAGE CLUB points/entries by clicking on the "Submit" button for Awards, or for Raffle entries on the KOLA ONLINE ADVANTAGE CLUB website. In the event that another member is already in the process of redeeming that particular prize/item, the member who "first" clicked on "Submit" will receive the item/prize.
b. In order to receive a prize or reward, winning members must contact KOLA 99.9 in accordance with instructions provided as part of this program, or provided online in connection with a specific offering, AND comply with the instructions provided by KOLA 99.9 personnel for verification and deduction of points/entries balance.
c. Failure to comply with the instructions may result in disqualification, at the sole discretion of KOLA 99.9. Any member that has a "winning" bid, and then fails to demonstrate they have the needed points/entries in their account following an audit, will not be entitled to receive the award/prize and, at the discretion of KOLA 99.9, will have their points/entries balance forfeited.
d. Member points/entry totals are subject to adjustment, and member accounts are subject to suspension or cancellation, by KOLA 99.9 for any activity which is in violation of any rules, or which is not in compliance with the intent of the program. KOLA 99.9 shall be the sole determiner in cases of suspected abuse or violation of KOLA ONLINE ADVANTAGE CLUB rules, and any decision it makes relating to adjustment of points/entries or termination of membership shall be final and binding. Members will be notified of any such adjustments through a posting in the history log (viewable in the Transactions section of My Account in the KOLA ONLINE ADVANTAGE CLUB Program or through an e-mail sent to the e-mail address in the member's account profile.) Should the member disagree with any points/entries adjustment or account suspension, the sole remedy will be withdrawal from the program.
e. KOLA 99.9 makes no representations whatsoever regarding the value, if any, of points/entries earned as a result of participation. KOLA 99.9 is under no obligation to provide members with any particular number of points/entry-earning opportunities or On Line Advantage Club.


6) Conditions

a. The act of participation in the KOLA ONLINE ADVANTAGE CLUB program by Members, winners and qualifiers constitutes permission to the KOLA 99.9, sponsor, and its agencies to use the member's name and/or likeness for the purposes of advertising and trade without further compensation.
b. Members may be required to sign a liability release prior to acceptance of an item. Pursuant to the liability release, Member will agree to hold Triton Digital, Anaheim Broadcasting, KOLA 99.9, affiliated companies and the officers, shareholders, directors, employees, agents and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the KOLA ONLINE ADVANTAGE CLUB Program.
c. KOLA 99.9, in its sole discretion, reserves the right to disqualify any person tampering with the entry process, the operation of the KOLA 99.9 websites, or is otherwise in violation of the rules. It further reserves the right to cancel, terminate or modify the KOLA ONLINE ADVANTAGE CLUB program if it is not capable of completion as planned, including infection by computer virus, bugs, tampering, unauthorized intervention or technical failures of any sort.
d. KOLA 99.9 reserves the right to make changes to the official KOLA ONLINE ADVANTAGE CLUB program rules at any time without warning. Notification of any revisions and their immediate effectiveness will be permanently posted to this page and to your personal messages on your log in page for one week. You are encouraged to revisit this page from time to time to ensure that you are aware of any and all rules. It is the individual player's responsibility to know and understand the content of the official rules at all time. By using the KOLA 99.9 KOLA ONLINE ADVANTAGE CLUB Program and participating in the contests herein, you are indicating your understanding and acceptance of all official rules.
e. KOLA 99.9 reserves the right to substitute an item of equivalent or greater value should an original item awarded and/or purchased by a player become unavailable. Point/Entry values associated with any award can be changed or modified at any time. All Awards offered will show quantity available and will be redeemed on a first-come, first-served basis. KOLA 99.9 reserves the right to limit the quantity a member can buy of any specific Award.
f. KOLA 99.9 reserves the right to disqualify any member from receiving a cash or prize award, where KOLA 99.9, at its sole discretion, determines that the player has misrepresented or used unintended/unannounced means to accumulate points/entries to purchase prizes.
g. Members may be asked to supply personal information. This information will not be sold to outsiders, but the aggregate information (without identifying players as individuals) will be used for marketing purposes by KOLA 99.9 and its affiliates.
h. KOLA 99.9 reserves the right to discontinue, suspend or modify this Program at any time, including the termination of all opportunities to use or otherwise redeem accumulated KOLA ONLINE ADVANTAGE CLUB points/entries. From time to time KOLA 99.9 may decrease or zero out the points in all KOLA ONLINE ADVANTAGE CLUB accounts. At those times all accounts will either start at zero points or be decreased by an amount or percentage to be determined by KOLA 99.9 and all unused points/entries will no longer be valid. Members will be notified if and when the points/entry deduction will take place.
i. No purchase is necessary. The KOLA ONLINE ADVANTAGE CLUB program is void where prohibited.
j. Failure to comply with the KOLA ONLINE ADVANTAGE CLUB program rules may result in a player's disqualification at the sole discretion of KOLA 99.9.
k. A written copy of the KOLA ONLINE ADVANTAGE CLUB rules is available during regular business hours at the KOLA 99.9 offices or by sending a self-address stamped envelope to:
