KOLA 99.9 FM
GENERAL CONTEST RULES (As of February 25, 2010)

The following are basic contest rules for any and all contests run on KOLA 99.9. Each contest may have its own special qualifications and eligibility rules, in which case a separate addendum to these rules will be available.

a.		All KOLA 99.9 contests are open to any listener unless specific age is required by type of contest, participating agency or advertiser or by law. No household can win more than a maximum of 1 prize in any given month. This includes any contest where listeners must qualify for a grand prize (i.e. qualifying will count towards the 1 prize maximum. All qualifying prizes must be claimed at the KOLA business office - 1940 Orange Tree Lane Suite 200, Redlands 92374 - within 30 days. KOLA 99.9 is not responsible for the inability of the listener to redeem concert tickets, movie tickets, theme park tickets or the like due to circumstances beyond our control such as cancellation of events, rides or showings of any kind. No substitutions will be made if the tickets or passes cannot be used by the winner.
b.		Each winner must provide The Company with a current, valid government issued California identification and a valid taxpayer identification number or social security number before any prize will be awarded. Any person winning over $600 in prizes from The Company will receive an IRS Form 1099 at the end of the calendar year and a copy of such form will be filed with the IRS.
c.		Any contest that involves KOLA 99.9 choosing a winner from entries, such as email contests ("Muffin Monday", "Office Of The Day" etc.) or any "KOLA Online Advantage Club" contest where entering through the KOLA Club section of the KOLA website is required, will not count towards the 1 prize maximum in any given month.
d.		There may be contest(s) where you can only have one winner per household per year. Additionally, there maybe contests where a listener may only be a contestant once per calendar month (even if that listener didn't win).
e.		Unless otherwise specified, all contests are conducted during general programming hours and contests are not conducted during programs that are carried through syndicated sources whether "live" or pre-recorded that are not in control of KOLA 99.9. During hours that carry a syndicated program (non-general programming hours) contest rules and or the mechanics of operating any contests are void and no winners will be accepted during these times.
f.		Winners may be required to sign a release, and all winners agree to allow KOLA 99.9 to use their name and/or likeness for promotional purposes without further compensation. All calls coming in to KOLA 99.9 are taped and may be aired on KOLA 99.9 without further approval.
g.		KOLA 99.9 is not responsible for problems occurring within the telephone system which either inhibit or increase the chances of winning a phone-in contest. In the event two or more phone lines are "tied together" during a phone-in contest, the entire call and all callers "tied together" will be disqualified and another caller will be taken.
h.		KOLA 99.9 can not award duplicate prizes to all parties in a "tied together" situation. In case of telephone line malfunctions, the decision of KOLA 99.9 will be final.
i.		Employees and agents of KOLA 99.9, Anaheim Broadcasting and their families, their advertising agencies or representatives are ineligible to enter contests on KOLA 99.9. Winners may not request substitution or other consideration from the originally awarded prize.
j.		All prizes, unless otherwise stated, must be picked up in person, and signed for by the winner at the KOLA 99.9 business office, located at 1940 Orange Tree Lane Suite 200, Redlands, CA. 92374, any weekday, between 9AM-5PM, within 30 days of winning. Holidays and weekends are included in this 30-day period. Designated winner's representatives will be allowed to claim prizes, provided prior arrangements have been made. If prizes are not picked up within the stated period, the winner waives their rights to the prize and said prize becomes the possession of KOLA 99.9 and may be awarded to another listener at KOLA 99.9 discretion.
k.		KOLA 99.9 will not notify winners of time remaining on prizes to be picked up. It is solely the winner's responsibility to claim their prize within the time limit specified. No prize will be mailed unless specified during contest or if special arrangements have been made. Prizes are non-transferable.
l.		KOLA 99.9 reserves the right to revoke prizes if the winner does not meet any or all eligibility requirements, even though an announcement may have been made on the air indicating that person was the winner.
m.		KOLA 99.9 may, due to the unavailability of prizes awarded, substitute another prize of equal or greater value for the prize originally won.
n.		KOLA 99.9 will not be responsible, or held liable for prizes awarded to an event that is canceled due to circumstances beyond our control. With respect to any disputes regarding contests, the decision of KOLA 99.9 will be final.
o.		KOLA 99.9, its agency, and representatives absolve themselves and shall be held harmless from any action or liability arising from any KOLA 99.9 contest or prize awarded in a contest. All contest entries become the property of KOLA 99.9.
p.		KOLA 99.9 may withhold any or all contest materials for any reason station management deems crucial to the completion of the contest, or for reasons relating to publicity and/or advertising. Unless otherwise stated, multiple entries by the same person will be discarded and may be deemed cause for disqualification. Additions or deletions to these rules are subject to change without notice.

Getaway & Play Days

Contest:	"Getaway & Play Days"
Dates:	Monday May 27, 2013 - Friday August 30, 2013
Times:	Weekdays during the hours of 7:00a, 11:00a, 3:00p, 7:00p (times are approximate and subject to change)
Prize:	Tickets to various Theme Parks / Concert Venues or other prizes
Grand Prize:	Option 1 *$10,000 cash and a two (2) year prepaid lease of a 2013 Camaro 2 door sedan. Prize is non-transferable.
Value Option 1:	Approximately $20,000.00
Grand Prize:	Option 2 *$10,000 cash and a $5,000 cash out value in place of the (2) year prepaid lease of a 2013 Camaro 2 door sedan. Prize is non-transferable.
Value Option 2:	Approximately $15,000.00
Taxes / 1099:	Winner must provide Anaheim Broadcasting (the company) with a current, valid government issued California identification and a valid taxpayer identification number or social security number before any prize will be awarded. Any person winning over $600 in prizes in a calendar year from The Company will receive an IRS Form 1099 at the end of the calendar year and a copy of such form will be filed with the IRS. *Prize money in any denomination to be delivered by check. Allow 6-8 weeks.
Eligibility:	Must be at least 21 years of age. Any potential winner, even if announced, is subject to verification. If potential winner does not meet all requirements, no prize will be awarded regardless of any announcement of a winner's status. All contestants (qualifiers) must be at least 21 years of age AND a licensed driver with a valid California Driver’s license. KOLA 99.9 FM is not responsible for the inability of the listener to enter through technical, mechanical, or other errors associated with entering said contest.
Online:	www.kolafm.com. KOLA Online Advantage Club Members may have access to winning information during this contest that may or may not be announced on air. No purchase necessary to join. Additionally, members may have the chance to "enter to win" or bid on a spot in the grand prize event through spending points.
How to Play:	During the contest hours (weekdays), listeners will be encouraged to be the 19th caller at 909-798-5600. If they are the correct caller and meet all the eligibility requirements, they will win the tickets/prizes available at that contest time. Plus, they will be entered as a qualifier into the Grand Prize Event on Saturday, September 7, 2013 (9a-11a) at Tom Bell Chevrolet. *Must be present to win. No proxies and no transfer of eligibility to another person. The person who qualified on air or online is the only person who can represent himself or herself at the Grand Prize Event. Not responsible for the inability of any qualifier to not be present to win at the Grand Prize Event location. In order to be eligible to win the grand prize, ALL ENTRANTS MUST BE A MEMBER OF THE KOLA ONLINE ADVANTAGE CLUB PRIOR TO AUGUST 30, 2013 5PM.* ONLY ONE WINNER PER HOUSEHOLD PER CALENDAR MONTH of any tickets that are associated with "Getaway and Play Days". ONLY A MAXIMUM OF 3 QUALIFYING ENTRIES PER HOUSEHOLD FOR THE GRAND PRIZE event ON SEPTEMBER 7, 2013 at Tom Bell Chevrolet at 800 Alabama Street, Redlands, Ca 92374 9a-11a. Listeners will have an additional chance to earn entry into the Grand Prize Event daily by entering a bonus code given out during the scheduled contest hours. From all those with the correct bonus code, additional qualifier(s) for the Grand Prize Event will be chosen and tickets or a prize may be awarded. At least one qualifier using this method will be chosen each weekday. Additionally, listeners will have a chance to “buy their way in” to the Grand Prize event through the use of KOLA Online Advantage Club points. No purchase necessary to join the club. At least 15 contestants will be chosen to enter the grand prize event using this method. However, using this method, no tickets will be awarded. The more points used, the more chances of being randomly chosen for the Grand Prize Event. Those randomly chosen from all entries using this method will only win one (entry) spot in the Grand Prize Event and will not be awarded any tickets associated with Getaway & Play Days. Only one qualifier per household per any method used per month. listeners may enter a bonus code each day during the contest to earn points, but can only qualify once per month for a total maximum of three qualifications per household for the grand prize event. All qualifying prizes (tickets) must be claimed at the KOLA business office – 1940 Orange Tree Lane Suite 200, Redlands 92374 – within 30 days of winning. Secret contest hours may be available to KOLA Online Advantage Club Members through the KOLA website at KOLAFM.COM. No purchase necessary. KOLA 99.9 may, due to the unavailability of prizes awarded, substitute another prize of equal or greater value for the prize originally won.
Exceptions:	The contest is an "exception" to the 1 prize per calendar month rule. That is, a person may win 1 other prize during a calendar month and will still be eligible to win once with ""Getaway & Plays Days".
Disclosure:	On Saturday September 7, 2013 between 9a-11a Tom Bell Chevrolet at 800 Alabama Street, Redlands, Ca 92374, KOLA 99.9 FM will randomly draw a winner from all the contestants in attendance. Contestants will be made up of all on-air qualifiers who are also members of the KOLA Online Advantage Club and KOLA Online Advantage Club Members who entered to win or bid on an entry to the grand prize event through the members section of www.kolafm.com. TO WIN THE GRAND PRIZE, ALL QUALIFIERS MUST BE A MEMBER OF THE KOLA ONLINE ADVANTAGE CLUB. IF THEY ARE NOT, THEY ARE INELIGIBLE TO WIN THE GRAND PRIZE. Must be present at the grand prize event to win. All registered contestants who have properly completed and signed a release form and have been verified, will be eligible for the Grand Prize: $10,000 plus a Two (2) year prepaid lease of a 2013 Chevrolet Camaro 2 door sedan. To receive a Two (2) year prepaid lease of a 2013 Chevrolet Camaro 2 door sedan, grand prize winner must satisfy credit worthiness/insurability criteria as determined by Ally Financial, Inc., and must execute a Ally Financial, Inc. lease agreement and comply with all terms thereof, including returning the leased vehicle at the end of the Two (2) year period. The grand prize winner is responsible for all taxes (and registration fees) and fuel and insurance costs associated with the leased vehicle. If the selected grand prize winner does not satisfy the applicable Ally Financial, Inc. credit worthiness/insurability requirements, KOLA 99.9 FM/Tom Bell Chevrolet will award the amount of $5000 as a substitute grand prize to said individual arranged solely by Tom Bell Chevrolet.

How to Enter:	*Starting on Friday 5/31/13 – Friday 6/21/2013 all* listeners will be encouraged enter to be Cindy Davis’ Office of the Day. If you have previously entered to be the Office of the Day you’re entry will still count towards the San Diego Getaway. The winning office is randomly selected from all entries. Each weekday during the promotion at varying times Cindy Davis will announce a new winner and their office. This winner will win lunch for four at Richie’s Diner (6/3 – 6/7) or Ray’s Pizza (6/17 – 6/21) and will automatically be qualified to win the San Diego Getaway. On (or after) 6/21 a grand prize winner will be randomly selected from *previous winners chosen within the dates of Monday, June 3rd, 2013 through Friday June 6^th, 2013 and between June 17th-June 21^st, 2013*

More:	KOLA 99.9 reserves the right to disqualify any member from receiving a cash award or prize award where KOLA 99.9, at its sole discretion, determines that the member has violated any of these rules, made any misrepresentation in connection with this program, or used any unintended/unannounced means to accumulate or redeem KOLA ONLINE ADVANTAGE CLUB points/entries.
Prize Awards:	All prizes must be picked up at the office (unless otherwise specified) of KOLA 99.9 located at 1940 Orange Tree Lane Suite 200, Redlands Ca 92374 unless otherwise specified upon point/entries redemption. Station hours are 8:30am to 5pm, Monday through Friday (except holidays). When picking up your prize, you must provide a valid California Driver's license or a government issued photo ID card, your prize confirmation print out and your social security card (if expressly stated). In addition, you MUST fill out a station provided tax/prize release form before your prize will be awarded. If a winner is unable to personally pick up a prize, said winner can assign a proxy to do so by sending a clear readable copy of their California Drivers License or government issued ID card, an original and complete tax/prize release form (provided to the winner in advance by the station) along with a signed and detailed letter of release authorizing a "specific person" to pick up a "specific prize". All signatures on these documents must clearly match the signature on the government issued ID. Furthermore, the proxy must provide proof of their identity (in the form of a valid California Drivers License or a government issued photo ID card) as identified in the release letter provided by the winner. If any of the proxy documents provided are suspect for any reason, Anaheim Broadcasting retains the right to delay release/delivery of the prize until the documents provided can be verified. If acting as proxy for other KOLA ONLINE ADVANTAGE Club Program members, you may only pick up prizes for up to 3 people TOTAL per day. Prizes must be picked up within 30 days of purchase - unless otherwise specified. Prizes not picked up within 30 days from the date of purchase will be forfeited and points/entries will not be refunded. Mailing options are at the sole discretion of KOLA 99.9 and will only be considered in extreme circumstances, or will be based on the prize size, weight, timeliness and value. Prizes valued at over $15, time-sensitive prizes, tickets and gift certificates must be picked up. Member is always responsible to keep his/her KOLA ONLINE ADVANTAGE CLUB account up-to-date with current information. If KOLA 99.9 deems that an individual has extreme circumstances and a prize is mailed out, KOLA 99.9 is not responsible if it is lost, stolen, misdirected, or damaged. The prize will not be replaced with future prizes nor will points/entries be refunded. Prize winners are responsible for any and all expenses associated with their travel to and from the KOLA 99.9 offices or other any location(s) associated with claiming a prize or award. KOLA ONLINE ADVANTAGE CLUB points/entries will not be credited or refunded for any reward, item or certificate which a Member claims but does not subsequently use. Member agrees to provide only true and accurate information to the KOLA ONLINE ADVANTAGE CLUB Program at all times. In conjunction with claiming any item associated with the KOLA ONLINE ADVANTAGE CLUB Program or spending any points/entries for items, Member agrees to verify that all information in their KOLA ONLINE ADVANTAGE CLUB account profile is up-to-date and accurate by providing a valid California driver's license or government issued photo ID that matches the information in their profile. Furthermore, regular profile updates are strongly encouraged. KOLA ONLINE ADVANTAGE CLUB award items are not transferable. Awards may not be substituted for cash or any other items or merchandise. All award claims are subject to verification by KOLA 99.9 Management. All decisions made by KOLA 99.9 Management are final. Prizes and awards will be awarded and distributed at the discretion of KOLA 99.9.
More:	Prize is non-transferable. Hotel guest room accommodations for winner and guest, double occupancy, 2 night based on availability. Not redeemable for cash, not transferable, may not be combined with another offer. Winner must use all prize components in a single visit. Winner may not substitute prize components. Gratuities are not included. Hotel guest may be asked present a major credit card at check-in to guarantee hotel charges not included in this prize. 1) Participation.
To participate in the KOLA ADVANTAGE program, visit http://www.kolafm.com/ or http://club.kolafm.com and sign up as a member. Limit one account per person and two accounts per household. Once signed up as a KOLA ONLINE ADVANTAGE CLUB member, you can earn entries/points by visiting the KOLA 99.9 website, logging on to the KOLA ONLINE ADVANTAGE CLUB site and doing any of the following:

a.		You can enter certain KOLA ONLINE ADVANTAGE Club Raffles, simply by signing up and clicking 'Enter' next to the contest listing.
b.		At various times during the day, other KOLA ONLINE ADVANTAGE CLUB Codes, words, or names may be announced or posted on the site. These must also be entered online before a designated time that will be posted on the site or announced on-air.
c.		Listen to KOLA 99.9 and log on to http://www.kolafm.com/ for dates and locations of KOLA 99.9 events where members can pick up KOLA ONLINE ADVANTAGE CLUB Bonus Codes
d.		All new members to the KOLA ONLINE ADVANTAGE Club, will be awarded 250 KOLA ONLINE ADVANTAGE CLUB points just for signing up!
e.		KOLA ONLINE ADVANTAGE CLUB entries/points may also be earned in a variety of other ways, at the discretion of KOLA 99.9. Members can tune into KOLA 99.9 for details on additional ways to accumulate entries/points, or log on to your KOLA ONLINE ADVANTAGE CLUB account http://www.kolafm.com/ or http://club.kolafm.com KOLA 99.9 reserves the right to alter the ways of earning entries/points at its discretion.
f.		Items in the KOLA Free Store or in "Raffles" are generally available at all times but may become, at a later date, unavailable except during specified times of the day, Monday - Sunday.
g.		To become and remain a registered KOLA ONLINE ADVANTAGE CLUB member, you must have Internet access and a valid e-mail account. KOLA 99.9 is not responsible for a member's inability to: connect to the Internet, log on to the KOLA ONLINE ADVANTAGE CLUB website, gain access to their KOLA ONLINE ADVANTAGE CLUB account or participate in any activity associated with this program, service outages or delays, computer difficulties or other technological glitches. The KOLA ONLINE ADVANTAGE CLUB program is free and no purchase is necessary.

2) Eligibility
Participation in the KOLA ONLINE ADVANTAGE CLUB program is open to all legal residents of Southern California and counties within the radio broadcast signal area (Los Angeles, Orange, Riverside, Ventura, and San Bernardino counties) who are 18 years of age or older, that possess a current and valid social security card. Those specifically not eligible are full or part-time employees of Anaheim Broadcasting (including former employees and their next of kin), other Southern California radio station employees and their immediate families, M2O Media, Triton Digital, and sponsors featured in the Program.

Participation is expressly limited to one account per person, two accounts per household. Points/Entries are non-transferable and may not be earned in advance. All duplicate accounts or suspected duplicate accounts are subject to cancellation without notification by KOLA 99.9. E-mail addresses may not be shared between players; therefore, each account needs to have a unique, valid e-mail address associated with it.

››		Only one (1) prize per household for the Contest. Only one (1) Raffle prize per household per given named month.
››		Accounts which have an excessive number of rejected e-mails may be automatically locked, preventing the player from accessing the KOLA ONLINE ADVANTAGE CLUB program until they contact KOLA ONLINE ADVANTAGE CLUB Tech Support and provide an alternate e-mail address. Repeated problems with e-mail delivery to a player may result in permanent suspension of the member's account.
››		Members are not authorized to access any other member's account for any reason, including but not limited to entering codes or redeeming KOLA ONLINE ADVANTAGE CLUB entries/points for prizes or awards, even if the other member is a family member. Members must personally enter codes and play the various KOLA ONLINE ADVANTAGE CLUB games and surveys themselves to legally accumulate points/entries.
››		No machines, scripts or automated services may be used to access any part of the KOLA ONLINE ADVANTAGE CLUB website, accumulate any points/entries, bid on prizes and awards, or in any other way participate in this program. Any attempt to saturate the KOLA ONLINE ADVANTAGE CLUB system's resources by making continuous automated requests to the system is not permitted, and is grounds for account suspension or termination at the sole discretion of KOLA 99.9.
››		KOLA 99.9 is NOT responsible for members missing out on points/entries due to, but not limited to, the following: forgotten username and/or password; problems, updates or maintenance which affects the KOLA ONLINE ADVANTAGE CLUB program at a particular time, Internet, browser, server, or computer problems in general, lost codes, bonus codes or coupons, forgotten, incorrectly entered or misspelled codes, etc.
››		Redemption of any points/entries suspected to have been made in such a fashion so as to not deduct the appropriate balance, may be disqualified (not accepted), at the sole discretion of KOLA 99.9.
››		The authorized account holder or member is defined as the natural person who is assigned to the e-mail address by an Internet access provider, online service provider, or organization that is responsible for assigning e-mail addresses or the domain associated with the submitted e-mail address.
››		KOLA 99.9 reserves the right to disqualify any member from receiving a cash award or prize award where KOLA 99.9, at its sole discretion, determines that the member has violated any of these rules, made any misrepresentation in connection with this program, or used any unintended/unannounced means to accumulate or redeem KOLA ONLINE ADVANTAGE CLUB points/entries.
››		Posting or sharing of KOLA ONLINE ADVANTAGE CLUB Codes, Bonus Codes, or other KOLA ONLINE ADVANTAGE CLUB points/entries earned among members is expressly prohibited. Violations will result in the termination of all violating account(s) and/or forfeiture of all accumulated points/entries with no compensation of any kind.
››		KOLA 99.9 reserves the right to use any listener feedback in any way they choose.

3) Prize Awards
Each item on the KOLA Free Store and Raffle prize pages will be assigned a point/entry value. Members may redeem accumulated KOLA ONLINE ADVANTAGE CLUB points/entries for free prizes and awards.

a.		All prizes must be picked up at the office of KOLA 99.9 located at 1940 Orange Tree Lane Suite 200, Redlands Ca 92374 unless otherwise specified upon point/entries redemption.
b.		Station hours are 8:30am to 5pm, Monday through Friday (except holidays). When picking up your prize, you must provide a valid California Driver's license or a government issued photo ID card, your prize confirmation print out and your social security card (if expressly stated). In addition, you MUST fill out a station provided W9 tax form before your prize will be awarded.
c. d.		If acting as proxy for other KOLA ONLINE ADVANTAGE Club Program members, you may only pick up prizes for up to 3 people TOTAL per day.
e.		Prizes must be picked up within 30 days of purchase - unless otherwise specified. Prizes not picked up within 30 days from the date of purchase will be forfeited and points/entries will not be refunded.
f.		Mailing options are at the sole discretion of KOLA 99.9 and will only be considered in extreme circumstances, or will be based on the prize size, weight, timeliness and value. Prizes valued at over $15, time-sensitive prizes, tickets and gift certificates must be picked up. Member is always responsible to keep his/her KOLA ONLINE ADVANTAGE CLUB account up-to-date with current information. If KOLA 99.9 deems that an individual has extreme circumstances and a prize is mailed out, KOLA 99.9 is not responsible if it is lost, stolen, misdirected, or damaged. The prize will not be replaced with future prizes nor will points/entries be refunded.
g.		Prize winners are responsible for any and all expenses associated with their travel to and from the KOLA 99.9 offices or other any location(s) associated with claiming a prize or award.
h.		KOLA ONLINE ADVANTAGE CLUB points/entries will not be credited or refunded for any reward, item or certificate which a Member claims but does not subsequently use.
i.		Member agrees to provide only true and accurate information to the KOLA ONLINE ADVANTAGE CLUB Program at all times. In conjunction with claiming any item associated with the KOLA ONLINE ADVANTAGE CLUB Program or spending any points/entries for items, Member agrees to verify that all information in their KOLA ONLINE ADVANTAGE CLUB account profile is up-to-date and accurate by providing a valid California driver's license or government issued photo ID that matches the information in their profile. Furthermore, regular profile updates are strongly encouraged.
j.		KOLA ONLINE ADVANTAGE CLUB award items are not transferable. Awards may not be substituted for cash or any other items or merchandise.
k.		All award claims are subject to verification by KOLA 99.9 Management. All decisions made by KOLA 99.9 Management are final.
l.		Prizes and awards will be awarded and distributed at the discretion of KOLA 99.9.

4) Taxes
Any valuation of the prize(s) stated above is based on available information provided to The Company, and the value of any prize awarded to a winner may be reported for tax purposes as required by law. Each winner is solely responsible for reporting and paying any and all applicable taxes related to the prize(s) and paying any expenses associated with any prize which is not specifically provided for in the official rules. Each winner must provide The Company with valid government issued California identification and a valid taxpayer identification number and social security number before any prize will be awarded. Any person winning over $600 in prizes from the Company will receive an IRS form 1099 at the end of the calendar year and a copy of such form will be filed with the IRS. If the prize includes travel, lodging, entertainment, or a special event, winner is responsible for all taxes, expenses, and gratuities not expressly included in and as part of the prize. Winner agrees to accept all blackout dates, space availability requirements, etc. established by The Company's prize partner(s), such as hotels, airlines, trains, ships, etc. Winner agrees that acceptance of any trip taken as a prize from The Company does so entirely upon their own initiative, risk, and responsibility. If the trip requires traveling outside of the United States, the winner, upon winning the prize, must have a valid US passport. If the winner does not have a valid US passport upon winning the prize, the trip may be forfeited.

5) How to Redeem Points / Entries to Receive Prizes

a.		A validated KOLA ONLINE ADVANTAGE CLUB Member redeems KOLA ONLINE ADVANTAGE CLUB points/entries by clicking on the "Submit" button for Awards, or for Raffle entries on the KOLA ONLINE ADVANTAGE CLUB website. In the event that another member is already in the process of redeeming that particular prize/item, the member who "first" clicked on "Submit" will receive the item/prize.
b.		In order to receive a prize or reward, winning members must contact KOLA 99.9 in accordance with instructions provided as part of this program, or provided online in connection with a specific offering, AND comply with the instructions provided by KOLA 99.9 personnel for verification and deduction of points/entries balance.
c.		Failure to comply with the instructions may result in disqualification, at the sole discretion of KOLA 99.9. Any member that has a "winning" bid, and then fails to demonstrate they have the needed points/entries in their account following an audit, will not be entitled to receive the award/prize and, at the discretion of KOLA 99.9, will have their points/entries balance forfeited.
d.		Member points/entry totals are subject to adjustment, and member accounts are subject to suspension or cancellation, by KOLA 99.9 for any activity which is in violation of any rules, or which is not in compliance with the intent of the program. KOLA 99.9 shall be the sole determiner in cases of suspected abuse or violation of KOLA ONLINE ADVANTAGE CLUB rules, and any decision it makes relating to adjustment of points/entries or termination of membership shall be final and binding. Members will be notified of any such adjustments through a posting in the history log (viewable in the Transactions section of My Account in the KOLA ONLINE ADVANTAGE CLUB Program or through an e-mail sent to the e-mail address in the member's account profile.) Should the member disagree with any points/entries adjustment or account suspension, the sole remedy will be withdrawal from the program.
e.		KOLA 99.9 makes no representations whatsoever regarding the value, if any, of points/entries earned as a result of participation. KOLA 99.9 is under no obligation to provide members with any particular number of points/entry-earning opportunities or On Line Advantage Club.

6) Conditions

a.		The act of participation in the KOLA ONLINE ADVANTAGE CLUB program by Members, winners and qualifiers constitutes permission to the KOLA 99.9, sponsor, and its agencies to use the member's name and/or likeness for the purposes of advertising and trade without further compensation.
b.		Members may be required to sign a liability release prior to acceptance of an item. Pursuant to the liability release, Member will agree to hold Triton Digital, Anaheim Broadcasting, KOLA 99.9, affiliated companies and the officers, shareholders, directors, employees, agents and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the KOLA ONLINE ADVANTAGE CLUB Program.
c.		KOLA 99.9, in its sole discretion, reserves the right to disqualify any person tampering with the entry process, the operation of the KOLA 99.9 websites, or is otherwise in violation of the rules. It further reserves the right to cancel, terminate or modify the KOLA ONLINE ADVANTAGE CLUB program if it is not capable of completion as planned, including infection by computer virus, bugs, tampering, unauthorized intervention or technical failures of any sort.
d.		KOLA 99.9 reserves the right to make changes to the official KOLA ONLINE ADVANTAGE CLUB program rules at any time without warning. Notification of any revisions and their immediate effectiveness will be permanently posted to this page and to your personal messages on your log in page for one week. You are encouraged to revisit this page from time to time to ensure that you are aware of any and all rules. It is the individual player's responsibility to know and understand the content of the official rules at all time. By using the KOLA 99.9 KOLA ONLINE ADVANTAGE CLUB Program and participating in the contests herein, you are indicating your understanding and acceptance of all official rules.
e.		KOLA 99.9 reserves the right to substitute an item of equivalent or greater value should an original item awarded and/or purchased by a player become unavailable. Point/Entry values associated with any award can be changed or modified at any time. All Awards offered will show quantity available and will be redeemed on a first-come, first-served basis. KOLA 99.9 reserves the right to limit the quantity a member can buy of any specific Award.

h.		KOLA 99.9 reserves the right to discontinue, suspend or modify this Program at any time, including the termination of all opportunities to use or otherwise redeem accumulated KOLA ONLINE ADVANTAGE CLUB points/entries. From time to time KOLA 99.9 may decrease or zero out the points in all KOLA ONLINE ADVANTAGE CLUB accounts. At those times all accounts will either start at zero points or be decreased by an amount or percentage to be determined by KOLA 99.9 and all unused points/entries will no longer be valid. Members will be notified if and when the points/entry deduction will take place.

Taxes / 1099:	Winner must provide Anaheim Broadcasting (the company) with a current, valid government issued California identification and a valid taxpayer identification number or social security number before any prize will be awarded. How to Play:	At each event that KOLA attends (usually described as an event with The KOLA Prize Caravan) a Bonus Points Card may or may not be handed out to anyone in attendance that is at least 21 years of age. No event is guaranteed to have a Bonus Points Card opportunity. Instructions on the Bonus Points Card will direct that person to the KOLA 99.9 website (www.kolafm.com) where a bonus code can be entered. If the correct bonus code is entered, that person's KOLA Online Advantage Club account will be credited with points and will be entered once for the grand prize of a trip for two to Hawaii. The more different Bonus Points Cards received with a different bonus code, (between Friday January 18^th , 2013 – Monday January 20th, 2014) the more potential entries will be added to that member's account. All Bonus Codes will expire. Usually within 7 days. However, they may expire in as little as 24 hours.
